- Queried 110,521 outpatient appointments and found a pattern: no-show rate rises about 7x with booking lead time (4.6% same-day to 33.0% at 31+ days); 8+ day bookings are 36% of volume but 57% of no-shows.
- Tested the SMS reminder effect with a lead-time stratified comparison: raw rates made SMS look harmful (27.6% vs 16.7%), but within each lead-time band recipients no-showed 2.8 to 7.3 points less (observational).
- Recommended a reconfirmation step for 8+ day bookings plus a measured experiment; scenario estimate (assumes 20% recovery) lowers the overall no-show rate from 20.2% to 17.9%.
